Set of guidelines how to improve existing integrated care initiatives

Policy report on how to improve existing integrated care initiatives (including facilitators and barriers for quality improvement in integrated care, recommendations). End user: decision-makers and policy-makers

State-of-the-art report on characteristics of integrated care initiatives

All partners responsible for partnering (and associated data collection) will provide a national report about results of the baseline assessment, including a list of detailed improvement objectives for each site. Scientific research paper on the characteristics of existing integrated care initiatives including a comparison of their performance with regard to patient-centredness, prevention orientation, safety, and efficiency in order to be able to stratify their performance to their characteristics and context in which they are being practiced.

Policy report on transferability and applicability of actions for improvement other contexts

Policy report on transferability and applicability of actions for improvement to other contexts and scientific paper on how to improve existing integrated care initiatives and starting-points for transferability and applicability of actions for improvement to other contexts (a paper ready to submit will be available by the end of the project).

Implementation strategy per site

Fourteen site-specific reports will be written describing the tailored sets of improvements, the outcomes of the process evaluations and the post-assessments. The main target group will be the implementation sites.

Dissemination package

Tailored package of dissemination strategies targeted at different audiences which will include the knowledge transfer tools defined in task 7.4.

Set of instruments and protocols for the utilisation of the EIT in the context of integrated care

This set will include protocols and instruments for the utilisation of the EIT in the context of integrated care, including a participatory protocol with instruments for designing, implementing and evaluating tailored sets with stakeholders and a protocol with instruments for the overarching methodological and analytical approach. This set will be added to the roadmap.

Report on commercial opportunities in education and IT sectors

Report on commercial opportunities in education and IT sectors to support the implementation of patient-centred, prevention-oriented, safe, and efficient integrated care.
