Obiettivo Most of Central American health systems are fragmented. There are various sectors, including often-unregulated private healthcare, providing first line health services with different access, quality and range of activities. This leads to problems of equity and efficiency. Contracting out is fashionable in Central America to improve State regulation of private providers. This concerted action will build an analytical framework of local experiences of contracting private first line health services by public actors. One of its main feature, will be to describe the effects of contracting on sustainability, equity and efficiency in the provision of services on the base of the perception of target population, in addition to those of public purchaser, and private providers. The final aim of this framework is to strengthen the state by giving better capacity of policies adaptations to regulate private providers.
