An intact complement system is of great importance in the defence against meningococci. Deficiencies of one of the components of the terminal route or the alternative route lead to an increased risk for developing meningococcal disease. The estimated attack rate of meningococcal disease in complement deficient individuals is 45-75 %, but a lower attack rate of 23 % was recently found in the Netherlands. This suggests that complement deficient persons are capable of developing immunity without being clinically ill. Preliminary data indicate that certain IgG Fc receptor polymorphisms on leucocytes of complement deficient patients are also of importance for the immunity against meningococci.
There is a very important ethnic background that determines the prevalence of IgG Fc receptor polymorphisms and the complement deficiency state in various populations. Data on the prevalence of complement deficiency and IgG Fc receptor polymorphisms often reflect the ethnic bias of western Europe and north America and are thus not applicable to Russia. In addition, the disease in complement deficient individuals is frequently caused by meningococcal serogroups (W135, X, Y, 29E, Z and non-groupable strains) that are relatively uncommon in the west-European and American population. Common meningococcal serogroups such as A, C, and B are seldom associated with complement deficiencies in Europe and America. Data on the prevalence of different meningoccocal serogroups, serotypes and subtypes in Russia is not yet available.
The aim of this project is to compare the findings in the Netherlands so far with the results of a prospective Russian study on the following issues: the prevalence of complement deficiency in patients with meningococcal disease by uncommon European (W135, X, Y, Z, 29E, or non-groupable strains) or common European serogroups in Russia; IgG Fc receptors polymorphism in late complement component deficient individuals as an additional risk marker for meningococcal disease; comparison of the quantitative serum antibody response to the tetravalent meningococcal polysaccharides vaccine of late complement component deficient individuals from Russia with vaccinated late complement component deficient individuals and vaccinated healthy controls from the Netherlands; comparison of the functional serum antibody response to the meningococcal vaccine by measuring opsonophagocytosis and bactericidal effects in late complement component deficient individuals from Russia with late complement component deficient persons and controls from the Netherlands.
